Enrollment Requirements:

All State and Casita Feliz required forms and information requested must be completed prior to enrollment.
It is of the utmost importance you provide a current immunization record at time of enrollment.
We ask that at the time of filling in your application and other forms you not leave any blank spaces. If the
question does not apply to you answer “N/A”  (Not Applicable), this way we know you were aware of the
question being asked.

The State of Arizona requires all of child’s immunization be current and a record provided.  If a child is exempt
from any vaccination due to medical or religious reasons, a letter from your doctor will be required.

Casita Feliz requires both parents to provide a valid identification. If legal custody issues exist, copies of
court documents must be presented at the time of enrollment and kept in child's file.
